Project Concept
The Health Department was migrating from a paper form system with all of the paper being kept in a warehouse. New laws dictated that they needed to move to a digital system. This system in particular was for nursing intakes and incident reporting used by the school nurses in the county. They needed an efficient system in order to accurately record their students’ information.

Research
The user research process was extensive. Over the course of a few months I held several interviews with the nurses that would be using this system, as well as administrators of the Health Department that would be managing this system. I studied the paper forms at length (the method for obtaining the data at the time), and came up with a solution to provide the best user experience possible.
Target Audience
Nurses in the local public school system and administrators from the county health department and their supervisors
Design Solution
Clean interface, minimal colors, and obvious form actions and errors help make this an easy experience. Each page displays the minimum amount of information needed, and the forms are split up into several pages to avoid having an overwhelming amount of data to enter at one time.
